2010 for participants to apply for the Trainer Re-Cert Clinic.

At this time, CYO will not offer a Trainers Clinic.
Please register with the league
prior to the Participants must sign in ½
hour prior to start of class.
Participants must be 16 years
of age or older to attend a clinic.

05 = the first two digits represent the year in which the clinic was taken
DS1 = Development Stream 1
or
CS1 = Coach Stream 1
C = Body Checking component
12345 = Your coaches number
H = Speak Out component

What Your Trainers Certification Number Means: 09-TK1-12345-H
All Trainer certification numbers have a universal expiration of August 31 - the year of expiry is reflected by the first 2-digits in the certification number
i.e. 09-TK1-12345-H in this case, the certification would expiry August 31, 2009.
09 = expiry date of August 31, 2009
TK1 = Level 1 Trainers Certification
12345 = your trainers number
H = Speak Out component

PRS = Preventive Services (Speak Out)

CERTIFICATION CROSSOVERS AND UPGRADES
How to crossover OMHA / GTHL / OWHA, etc. coaching / trainer certifications: Forward a copy of your current coach/trainer certification and current Preventive Services certification, along with your complete address (needed for the database and to send a new card) and the $8.00 crossover fee to the CYO Office. Information will be forwarded to the Alliance Office for processing. The Alliance will then mail out the new card directly to the participant.
