Police Check Procedures

All CYO Bench Personnel and On-ice Helpers are required to get a Police Check every four years. To obtain a Police Check, please follow the procedures below. Deadline is November 30^th of the current season.

1.     All Bench and dressing room personnel are required to get a Police Check every four years. To obtain a Police Check, please follow the procedures below. Deadline is November 30th of the current season.

2.     The CYO Hockey Office will issue you a signed and witnessed Police Check form, which can be picked up during regular scheduled hours. The League Administrator will sign the form.

3.     The two-sided completed form must be signed and dated by the applicant and delivered to the St. Catharines Police Department for processing. The fee is $15.00 and must be paid at the police station.

4.    The Police Department will instruct you as to when the Police Check will be ready for pickup. Two pieces of ID are required. One must include a picture.

5.    After picking up your completed Police Check from the Police Department, put it in a sealed envelope and deliver it to the CYO Hockey Office during regular scheduled hours.

6.    Once delivered, the CYO Representative, will review the Police Check.

Office Location

We are located at the rear of Jack Gatecliff Arena. You must enter our office from the outside of the arena (east side). We are across the street from the motorcycle shop.
